Loss Claim Forms This page is provided to assist JMS Insurance, Inc. clients with the convenience of using the internet for filing Loss Claims for property and auto listed on their existing policies. It is our policy to be in contact with you, the insured, within one (1) business day of receiving your submitted Loss Claim Form to verify your loss.  Please understand that any loss claims made via this site can not be processed until our Loss Claims Department has been in contact with you, the insured. If you, the insured, have not heard from our Home Office within one (1) business day of submitting your loss claim form for your existing policy, please contact our Home Office at (574) 586-7892 during our regular business hours of Monday through Friday -  9 am to 5:30 pm CST.  Although the internet offers many wonderful conveniences, it may be possible that your requested change(s) were not received.
